Wet Signature Enforcement & Authorized Signer Dropdown Now Live
New
The NHC Portal now enforces fresh handwritten signatures on key care documents and provides a controlled dropdown for selecting authorized in-person signers. Auto-generated signatures have been disabled for the affected documents, and both client and staff signatures are required before a submission is considered complete.
Forced Manual Signature Workflow — The Home Care Service Plan and Service Visit Record now require handwritten signatures from both the client and an authorized staff member on every submission. Documents remain marked Incomplete until both signatures are manually provided.
Authorized Signer Dropdown — A Manager Name and Manager Signature dropdown has been added to Admission Documents, Supervisory Reports, and Satisfaction Surveys. Restricted to authorized in-person staff only.

Full Audit Trail Logging Implemented for Document Signature Workflow

The portal now records a complete audit trail for all document signature activity, capturing every action from viewing through final submission. Ensures full accountability and traceability for HIPAA compliance.
Signature Event Logging — Logs all signature-related events including when a signature is added, updated, removed, or submitted. Each event is tied to a timestamp and signer identity.
Document Activity Tracking — Document-level events are tracked across the full lifecycle including when a document is viewed, downloaded, uploaded, modified, and finalized.

Admin User Accounts Provisioned for Signature Dropdown Access
Improved
Admin user access was set up for in-person staff so they appear in the approval form signature dropdowns. Two new accounts were created, one inactive account was reactivated, and one existing account was confirmed active.
New Accounts Created — Two new admin accounts were created and activated. Each user received a welcome email with a password setup link and will automatically appear in the admin signature dropdown upon first login.
Reactivated Account — One previously inactive account was reactivated with admin-level permissions restored.

User Email Redaction and Flexible Phone Validation Shipped
Improved
Email addresses are now masked on all generated and exported documents. Phone number validation has been relaxed to accept a wider range of formats without blocking submission.
Email Address Redaction — Email addresses are partially masked on all generated PDFs and exported documents. The full email remains accessible internally for system processing.
Relaxed Phone Validation — Now accepts mobile, landline, and international formats including inputs with spaces, dashes, or parentheses.

Admission Document Recipient Field Auto-Population Bug Fixed
Fixed
The booklet recipient field was being automatically populated with the case manager on file instead of remaining editable. The field has been corrected so it remains editable and can be completed directly by the intended recipient during signing.
Root Cause — The recipient field was pre-filled with the assigned case manager's information on two triggers: when admission documents were resent and on accounts created after the portal migration.
Resolution — The two affected authorization fields were reconfigured to accept free text input from both clients and admins. The auto-population logic was removed.

Authorization Document Downloads Restored
Fixed
Authorization documents in the client portal were failing to download across all accounts. The loading button would spin indefinitely before the session automatically logged out.
Root Cause — The Authorization document download function failed to generate files on request, confirmed as a backend document generation issue.
Resolution — Fixed by the development team and verified on affected client accounts. All old and new documents across affected accounts were restored and made available for download.

Apply Now Button Restored on Home Care Portal

The "Apply Now" button on the Nova Home Care Services website stopped working for users attempting to access the Indiana application flow at novahomecareservices.com → Apply Now → Indiana.
Root Cause — A temporary service interruption tied to a scheduled system update caused the form to briefly lose access to required resources.
Resolution — The form recovered automatically once the system update completed. No manual intervention was required.
